Abstract

Odontoma is a benign odontogenic tumor of mixed origin (epithelial and mesenchymal). It is sub-classified in two types:complex and compound. There is no gender predisposition, and  are often detected in the first two decades of life through routine examinations or changes in dentition. Odontomas are generally asymptomatic, they possess limited andslow-growing potential, but may occasionally reach considerable sizes, causing expansion of the cortical bone. The treatment for odontomas is surgical excision. They exhibitfavorable prognosis and recurrenceare rare. The purpose of this report is present the case of a 32-year-old female patient who sought the service of Maxillofacial Surgery at the Regional Hospital of Santa Maria for evaluation of a jaw injury revealed in a routine radiographic examination. On physical examination, an volume increase was observed in the right mandibular angle region on the inferiorvestibule. Imaging examination revealed a hyperdense lesion located in the region of teeth 48 and 47; with bulging buccal and lingual cortical bone, and associated impacted tooth (47). The diagnostic hypothesis was a complex odontoma. The patient was underwent an incisional biopsy, and histopathological analysis confirmed the diagnostic hypothesis. Partial removal of the lesion was performed under general anestesia - the lower portion being left in contact with the mandibular base. The final diagnosis was a complex odontoma. After one year of follow-up, no recurrence was reported. Partial removal of a giant complex odontoma proved to be an effective alternative, avoiding complications resulting by total removal.
